Company Overview

Florida Health Care Plans (FHCP) is a large multi-specialty group HMO that serves as both the insurance and the provider. We currently serve more than 100,000 members at 32 facilities and 6 Extended Hours Care Centers located throughout Volusia, Flagler, Seminole, Brevard, and St. Johns counties. Our outpatient, preventative care model has been successful for 51 years and counting.

Our state-of-the-art facilities are designed as multi-specialty hubs that contain primary care and other specialties that may include an Ambulatory Surgical Center, Bariatrics, Behavioral Health, Cardiology, Diabetes Education, Endocrinology, General Surgery, Hand Surgery, Infusion, Neurology, Occupational Health, Ophthalmology, Pain Management, Pharmacy, Rheumatology, Sports Medicine, and Wound Care. Lab, Pharmacy and X-Ray services are all onsite.

Founded in 1974, Florida Health Care Plans (FHCP) is a pioneering provider of affordable health plans and comprehensive health care services, available to individuals, families, employer groups, and Medicare eligible individuals. Headquartered in Daytona Beach, FHCP serves more than 100,000 members throughout Brevard, Flagler, St. Johns, Seminole, and Volusia Counties.

An affiliate of Florida Blue, and part of the GuideWell family of health solutions companies, FHCP offers access to a provider network of more than 9,500 doctors, specialists, and hospitals. FHCP also offers integrated care services at its community-based facilities, including primary and multispecialty care, radiology, ultrasound, laboratory, in-house pharmacies, as well as extended hour and Ambulatory Surgery Center services. FHCP is an independent licensee of the Blue Cross Blue Shield Association.
